Home Health Aide (Somerset County)

Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.

This position is a part time role and will serve primarily in the Somerset County Area.


+ Appropriately record all required documentation via the Telephony System accurately and in a timely manner.

+ Communicate appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patients condition to appropriate professional staff.

+ Comply with all safety policies and procedures, including standard safety precautions, oxygen safety precautions and blood borne pathogen exposure procedures.

+ Identify, develop and participate in process improvement opportunities within the home health agency that will enhance the quality of services we provide.

+ Independently provide safe and direct personal care, such as bathing, grooming, simple dressing changes, catheter bag changes and medication reminders, to multiple patients in a day in a home setting under the supervision and direction of a licensed professional as defined in the Home Care Aide Competency Evaluation Program.

+ Primarily assigned to one geographic area for coverage, but additional areas of coverage may be needed due to staffing levels and patient need.


+ High school diploma or equivalent.

+ One year of relevant experience in a healthcare setting.

+ Must have successfully completed a state established or other training program and competency evaluation program or state licensure program that meets the federal regulation for home health aide services (484.4 & 484.36). This will typically be completion of a nursing assistant or certified nursing assistant (CNA) training program.

Licensure, Certifications, and Clearances:

Registration with Nurse Aide Registry preferred. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
